Understanding the Levator–Müller’s axis within the multilamellar structure of the upper eyelid

When performing upper blepharoplasty, appreciating the true anatomical lamellae and the levator–Müller’s complex is critical. Subtle misjudgment in dissection or tissue handling can lead to ptosis, loss of natural crease, or even distorted blink mechanics.

đź§  The Three Lamellae of the Upper Eyelid

Surgeons must conceptualize the upper eyelid as three lamellar layers:

1. Anterior Lamella:

Skin: The thinnest in the body.

Orbicularis oculi muscle: Pretarsal and preseptal components.

This is the layer primarily involved in skin excision during blepharoplasty.

2. Middle Lamella:

Orbital septum: Fuses with the levator aponeurosis ~10–12 mm above the lid margin.

Preaponeurotic fat pads: Medial and central compartments (lateral pad is debatable).

Entry into this layer allows for fat contouring, but requires careful dissection to preserve deeper structures.

3. Posterior Lamella:

Levator palpebrae superioris aponeurosis

Müller’s muscle

Tarsal plate

Palpebral conjunctiva

These structures are critical to eyelid height, tone, and function.

Understanding these planes is fundamental to safe and effective blepharoplasty.


🧬 The Levator & Müller’s Muscle – Deep Functional Anatomy

Levator Palpebrae Superioris (LPS):

Origin: Lesser wing of sphenoid, superior to the optic canal.

Pathway: Passes over superior rectus, transitions to aponeurosis at orbital rim.

Insertion:

Anterior fibers to orbital septum and subcutaneous tissue

Main aponeurosis attaches to superior tarsal plate

Fibrous slips to skin form the supratarsal crease (~8–10 mm above lash line)

Müller’s Muscle:

Smooth muscle, sympathetically innervated, contributes 1–2 mm of eyelid elevation.

Originates from deep surface of the levator; inserts onto the upper border of the tarsus.

Works synergistically with LPS to maintain lid tone, particularly in primary gaze.

🛠️ Operative Implications in Upper Blepharoplasty

âś… Correct Layer Entry:

Incision through anterior lamella should preserve pretarsal orbicularis for scar concealment.

Entry into the middle lamella to access fat pads must be high enough to avoid aponeurotic trauma.

⚠️ Avoiding Functional Complications:

Disruption of the levator aponeurosis or Müller’s muscle can lead to iatrogenic ptosis.

Over-resection or cautery too deep may scar or weaken the aponeurosis.

Subtle injury may not present immediately — delayed ptosis can emerge weeks post-op.

🔍 Hidden Ptosis Detection:

Always evaluate levator function pre-op (normal ≥12 mm).

Mild ptosis may be unmasked after skin excision.

Consider levator advancement or Müller’s muscle-conjunctival resection in selected cases.
